Stroke Community Navigator - Mat Leave
Location: Greater Sudbury
Type: Full-Time, Contract
We are seeking a compassionate and skilled Stroke Community Navigator to cover am 18-month maternity leave. In this role, you will support individuals following a stroke by providing system navigation, education, advocacy, and community re-engagement services. You will work collaboratively with healthcare partners, including Health Sciences North, and engage with clients at both ICAN and hospital sites.
What We’re Looking For:
- Current membership in good standing with a regulated health professional college under the Regulated Health Professions Act, 1991.
- Experience working with stroke survivors in acute, rehabilitation, and/or community care settings.
- Demonstrated client-centered approach that promotes independence and self-advocacy.

What We’re Looking For:
Post-secondary education in Recreational Therapy, Activation, Health Promotion, Gerontology, or related field. Alternatively, experience working in recreational therapy within the last two years.
Group facilitation skills, with emphasis on social, recreational, and exercise activities.
Strong interpersonal, communication, and problem-solving skills.
