The Outreach program provides assistance with personal care and activities of daily living to individuals throughout the City of Greater Sudbury and Sudbury West (including Espanola, Massey and Manitoulin Island.
Staff are available at pre-determined times to provide assistance to individuals with physical disabilities. Services are designed to meet the individual's needs at home, work or an educational facility.
Outreach services are available at different intervals throughout the day. Hours of service extend from 6:00 a.m. to 12:00 midnight, 7 days per week, 365 days a year.
Eligibility:
- Clients must be sixteen years or older
- Require assistance with activities of daily living
- Able to have medical needs met, by the existing community health services, on a visitation basis
- Able to direct their own care
- Able to live safely unattended
Services Available:
- Personal hygiene
- Bathing/showering and washing
- Assistance with rising and retiring
- Dressing/undressing
- Tracheotomy care where the needs if the person are stable
- Bowel and Bladder routine
- Assistance with eating
- Essential communication
- Positioning and transferring
- Assistance with mobility
- Exercising
- Meal preparation
- Light house cleaning/home management
